The Lead Care Manager (LVN) works in collaboration and continuous partnership with chronically ill or "high-risk" members to coordinate their care. This involves engaging eligible members, overseeing the provision of ECM services, and implementing care plans to ensure a seamless and supportive healthcare experience.
Responsibilities
- Coordinate with individuals and entities to ensure a seamless experience for members and non-duplication of services
- Engage eligible members and oversee provision of ECM services
- Offer services where the member lives, seeks care, or finds most easily accessible
- Connect members to other social services and supports, including transportation
- Advocate on behalf of members with health care professionals
- Use motivational interviewing, trauma-informed care, and harm-reduction approaches
- Coordinate with hospital staff on discharge plans
- Monitor treatment adherence, including medication
- Provide health promotion and self-management training
- Create and promote adherence to a care plan developed in coordination with the member, primary care provider, and family/caregiver(s)
Requirements
- Valid CA License Vocational Nurse (LVN) License
- Reliable means of transportation
- Valid driver’s license, proof of insurance, and a good driving record
- Proof of Negative TB Test
- BLS/CPR certification
- Completion of Live Scan Fingerprint/Background check
- Proficiency in communication technologies (email, cell phone)
- Highly organized with the ability to keep accurate notes and records
- Excellent analytical, problem-solving, and prioritization skills
- Excellent verbal and written communication skills
- High-level interpersonal skills
- Effective computer skills, particularly Microsoft Office, Excel, PowerPoint, Word
Nice to Have
- Experience with Health IT systems and reports
- Local knowledge about and connections to community health care and social welfare resources
- Bi-lingual (Chinese, Mandarin, Spanish)
