Hospice Social Worker

About This Role

Provide medical social services to hospice patients and their families, conducting psychosocial assessments and participating in interdisciplinary group meetings. This role involves observing and reporting patient changes, maintaining clinical records, and coordinating visits.

Responsibilities

• Provide medical social services to the patient and/or family according to the Plan of Care
• Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program
• Participate in IDG meetings and activities and the development of the patient plan of care
• Observe, record, and report changes in the patient's emotional and social factors that are being affected by the patient's illness
• Maintain and submit written clinical records as deemed by Care 360 Hospice
• Confirm weekly, the scheduling of visits with the patient's case managers to facilitate coordination of other staff visits
• Assess the patient's and family member's ability to cope with the patient's passing
• Act as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient's health problems

Requirements

• Licensed Social Worker (LSW) in the state of practice
• Bachelor's Degree from a school of Social Work accredited by the Council of Social Work Education
• Current Driver's License
• Vehicle insurance
• Access to dependable transportation

Qualifications

• Bachelor's Degree from a school of Social Work accredited by the Council of Social Work Education, Masters degree preferred.
• 1+ year of social work experience in a healthcare setting preferred

Nice to Have

• Master's degree in Social Work
• One year of social work experience in a healthcare setting
