This Homecare RN role is a dynamic position for a full-time Registered Nurse to join a home care team, working directly with an interdisciplinary team to provide patient care in a home setting.
Responsibilities
- Work directly with an interdisciplinary team in the homecare setting
- Communicate effectively with interdisciplinary team members and family members
- Care for patients one at a time
- Help patients become independent with their treatments and self-management of chronic illnesses
Requirements
- 1 year RN acute care experience
- Valid NYS RN Licensure
- Valid NYS DMV Licensure
- Organized
- Able to teach
- Willing to travel from patient to patient
- Strong communication skills
- Strong critical thinking skills
Qualifications
- 1 year prior experience in acute care, rehab, or homecare
