Home Health Aide - CNA

About This Role

The Home Health Aide is responsible for providing personal care and support to home health patients, assisting with daily living activities, and observing and reporting changes in patient conditions to the case manager.

Responsibilities

• Provide personal care and hygiene to home health patients, including toileting and personal appearance.
• Assist patients with transfers, exercise, and ambulation.
• Turn and position bed-bound patients.
• Assist with Range of Motion (ROM) appropriate for the patient as directed by the RN or Therapist.
• Assist patients with meals and nutritional support.
• Change simple non-sterile dressings or external bags per state and federal regulations.
• Change bed linens and straighten patients' environment.
• Observe and report changes in patient conditions and needs to the case manager.
• Participate in interdisciplinary team meetings to coordinate patient care, exchange information, and problem-solve.
• Provide documentation of patient visits on the clinical record accurately and timely.

Requirements

• Certified Nursing Assistant (CNA) license in Washington
• Current CPR certification
• Ability to read, write, and speak English
• Working knowledge of home health aide principles, standards, and applications
• Knowledge of medical terminology

Qualifications

• High school diploma or GED highly preferred.
• One (1) to three (3) months related experience and/or training highly preferred. Preferred: 1 year Previous experience as a nurse’s aide, home health aide, orderly, or related experience in an acute, rehabilitative, or long term care facility, or home health/hospice organization.
