Home Care Intake Worker/SW

About This Role

This role involves evaluating applicants' home care needs, developing and initiating home care service plans, and advocating for consumers to ensure they receive eligible benefits and services.

Responsibilities

• Conduct intake need assessments and develop consumer-centered care plans with consumers, caregivers, and families
• Initiate and coordinate services rendered by providers
• Advocate for consumers in obtaining benefits and services for which they are eligible
• Create case records in compliance with regulations and OCES standards
• Make initial home visits for determination of program/service
• Participate in interdisciplinary activities
• Provide back-up support for Information & Referral Specialist performing general intake and providing information and referral

Requirements

• Bachelor's degree in social work or related field
• Ability to obtain information of a personal nature through interviews
• Ability to respond appropriately to emergency situations involving abuse and/or neglect of elders
• Ability to communicate effectively, both verbally and in writing
• Computer use proficiency

Qualifications

• Bachelor's degree in social work or related field
• One year of experience in the social services field preferred

Nice to Have

• Current Social Work License
• One year of experience in the social services field
• Considerable knowledge of social work, social, emotional, and physical processes of aging
