Provide direct patient care as a Home Health Aide or Certified Nursing Assistant for a long-term 24-hour case, ensuring safety and comfort for fall-risk and dementia patients.
Responsibilities
- Assist with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, feeding, and mobility
- Provide care for a fall-risk patient with close attention to safety
- Assist with transfers, repositioning, and mobility
- Provide companionship and emotional support
- Drive patient to appointments and run errands as needed
- Follow the patient’s individualized care plan
- Maintain patient privacy, dignity, and comfort at all times
- Communicate clearly with the care team regarding any changes or concerns
Requirements
- Valid HHA Certificate or CNA License (or ability to pass HHA competency test)
- Experience with Dementia and Alzheimer’s Patients
- Valid CPR Certification
- Negative TB skin test or current chest X-ray
- Level 2 Background Clearance
- Reliable transportation
- Valid Driver’s License with active auto insurance
- Ability to work independently and follow care instructions
