The Family Resource Navigator identifies and connects with community resources and partners to create a Family Resource Center, providing support to agency participants and community members through family management and referral systems navigation.
Responsibilities
- Offer rapid response (within 48-72 hours) when a family requests assistance
- Listen and provide confidential emotional support
- Help families with goal-setting and empower informed choices
- Assess families’ needs and strengths, collecting assessment and referral information
- Provide peer support in one-on-one and group settings
- Role model and coach families on identifying and accessing support systems
- Refer and link families to community resources
- Help families understand mental health evaluation, diagnosis, and treatment processes
- Discuss options in an unbiased and culturally sensitive manner, explaining processes, rights, responsibilities, and timelines
- Offer guidance with forms and paperwork, and support transition, safety, and reentry planning
- Identify relevant benefits programs (e.g., Medicaid, SNAP) and support enrollment
- Engage in school or community outreach and awareness events
- Identify and nurture relationships with community resources and providers, maintaining an updated list
- Coordinate FRC meetings, workshops, and trainings (e.g., parent information programs, parent education, family health screenings)
- Purchase and inventory consumables and supplies to keep the FRC stocked and organized
- Promote the Family Resource Center/PCCT by communicating with referral sources and groups that interact with families
- Create and liaise with a Parent Advisory Committee for ongoing needs assessment and quality improvement
- Attend required trainings, including Standards of Quality for Family Strengthening & Support
Requirements
- Firsthand experience navigating systems on behalf of child, youth, or young adult with health challenges and working with families
- Familiarity with community and regional resources
- Understanding of relevant systems, services, and processes (e.g., TANF, Medicaid enrollment)
- Strong interpersonal skills and empathy
- Understanding of and ability to maintain family confidentiality
- Familiarity and ability to work effectively with families of diverse racial, ethnic, socioeconomic, and cultural backgrounds
- Strong written and spoken communication skills, including familiarity with relevant languages for your region
- Ability to collaborate and network with community organizations and groups
- Active involvement in other community councils, advisory boards, or parent and family groups
- Formal training and/or work experience within the social service/case management field or training specific to navigating community service systems
- Ability to lift 25 lbs.
Nice to Have
- Bachelor’s degree in the social services or other relevant field
- Experience in human services in the public sector
- Skill in interacting with human service providers and agency leaders
- Understanding of the role of evaluation in social services
