The Family Partner II collaborates with families to navigate social services, mental health, and educational systems, providing support, guidance, and advocacy. This role involves developing individualized care plans, connecting families with resources, and monitoring their progress to enhance their well-being.
Responsibilities
- Collaborates with families to identify strengths, needs, and goals
- Supports families in navigating mental health, education, and social service systems
- Assists in developing and implementing individualized care plans
- Provides peer-based support and models self-advocacy
- Connects families to appropriate community resources and services
- Offers guidance on accessing benefits, educational supports, and treatment options
- Advocates for family voice in service planning and decision-making
- Partners with community agencies to coordinate care and ensure service alignment
- Conducts needs assessments and documents family progress
- Monitors care plan implementation and adjusts supports as needed
Requirements
- High School diploma or GED
- Ability to articulate the experience and perspective of a parent/caregiver or relative of a child with complex needs involving mental health, child protective, or probation services (typically acquired as a primary caregiver)
Qualifications
- High School diploma or GED required.
- Up to three (3) months of experience.
Nice to Have
- Ability to speak, read, and/or write another language
- Therapeutic crisis intervention and agency Wraparound training
- Basic computer word processing skills
- Diplomacy in resolving conflicts and integrating divergent perspectives
