About This Role

This role delivers psychotherapeutic services to children, adolescents, and families using the Family Centered Treatment model, focusing on healing and growth in client-centered care. The specialist provides services in clients' living environments, ensuring comprehensive support and coordination with treatment teams.

Responsibilities

  • Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements
  • Complete FCT Checkoffs and other internal trainings within designated deadlines
  • Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus
  • Provide 24/7 on-call coverage to a caseload of families and children
  • Complete all required Clinical Records and FCT documents and ensure timely entry/upload in the Electronic Health Record
  • Develop, revise, and implement Service Plans in coordination with the Treatment Team Process
  • Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan
  • Provide psychoeducation as indicated in the Service Plan
  • Conduct interventions with the family aligned with the FCT model, facilitating movement through treatment phases and completing fidelity documents
  • Assist families and children to establish weekly goals and identify steps to reach them
  • Make referrals to services and resources on behalf of the families and children
  • Develop and coordinate discharge plans based on treatment, indicating aftercare needs and plans
  • Coordinate and oversee initial and ongoing assessment activities
  • Consult with identified medical and non-medical providers, engaging community and natural supports
  • Ensure linkage to the most clinically appropriate and effective services, including arranging psychological and psychiatric evaluations
  • Provide and coordinate behavioral health services and other interventions for youth or family members with other professionals
  • Monitor and document the status of the consumer’s progress
  • Participate in Quality Assurance/Quality Improvement activities

Requirements

  • Minimum of a Bachelor's Degree in human services or related field
  • Two years’ experience working with youth or families
  • NC, VA- QMHP or ability to register as a QMHP (North Carolina, Virginia)
  • MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi

Qualifications

  • Bachelor's Degree in human services or related field
  • 2 years’ experience working with youth or families
