About This Role

Manage cases for elder victims of crime, including comprehensive needs assessment, service plan development, and coordination of care. Advocate for client rights and connect them to essential community resources.

Responsibilities

  • Complete client intake to determine eligibility for programs and services
  • Perform case management, including assessment of client needs and development of individualized service plans
  • Assist with crime victim compensation claims and provide information and referral services
  • Assist in meeting needs for shelter, housing, sustenance, medical, mental health, and substance use treatment
  • Provide victim advocacy and information about crime victims' rights within the criminal justice process
  • Connect clients to services for education, job training, employment assistance, and life skills training
  • Connect clients to transportation assistance and childcare services when needed
  • Provide 24-hour response to client emergencies, including evenings and weekends
  • Connect clients to legal services, including assistance with family and civil matters related to victimization
  • Determine entitlement eligibility through social/financial assessments and assist in accessing publicly funded programs
  • Visit clients at their place of residence
  • Assist Program Director in collecting client services and statistical information for grant/board reporting
  • Distribute information and education about program services, victim rights, and elder abuse prevention
  • Document services provided and client progress through clear and appropriate case notes

Requirements

  • Bachelor's degree in social work
  • 2 years relevant experience in case management
  • Ability to work effectively with the elderly
  • Strong interpersonal skills and flexibility
  • Effective communication skills, both verbal and written
  • Ability to work independently with little or no direction/follow-up
  • Knowledge of community programs for the elderly
  • Florida Driver's License, excellent driving record, and access to vehicles

Qualifications

  • Bachelor's degree in social work
  • 2 years relevant experience in case management
