About This Role

This role supports adults with intellectual and developmental disabilities by providing essential direct support in their homes and community, focusing on independent activities of daily living and maintaining a healthy, safe living environment.

Responsibilities

• Work with adults with developmental disabilities in their homes, requiring significant supports to maintain a healthy and safe living environment
• Provide independent activities of daily (IADL) supports, especially during disaster or emergency situations
• Create a learning environment where participants can learn to live independently in their home and community
• Design instruction methods based on measurable objectives, maintain records of participant progress, and analyze barriers to learning
• Travel and work with participants in their homes and in the community
• Coordinate individualized service delivery to participants
• Act as a liaison to families, care providers, case managers, and community resources
• Plan, implement, and coordinate clients' overall program plan
• Provide one-to-one support with independent activities of daily living (ADLs) including personal hygiene, continence management, dressing, feeding, ambulating, companionship, transportation, meal preparation, household management, medication management, communication, and financial management
• Ensure maintenance of client emergency information and client records
