The Hospice Continuous Care LPN identifies physical, psychological, social, and spiritual needs of patients and families under the supervision of an RN Case Manager. This role involves providing direct skilled bedside nursing care and documenting interventions.
Responsibilities
- Identify physical, psychological, social, and spiritual needs of the patient and family
- Work under the supervision and direction of the RN Case Manager
- Initiate appropriate interventions in support of the patient and family
- Follow a comprehensive and responsive plan of care
- Provide direct skilled bedside nursing care to patients
- Ensure and document care and interventions provided
- Document appropriate use of medication, durable medical equipment, and supplies
Requirements
- NJ Nurse License
- Minimum one (1) year of clinical experience
- Understanding of Hospice philosophy and needs of terminally ill
- Strong problem solving and critical thinking skills
- Excellent Communication Skills
- Ability to travel within the state of NJ
