Community Health Worker Engagement Specialist

About This Role

This role will connect diverse, underserved, and vulnerable populations to Strive Health’s interdisciplinary care model. The focus is on building trust and promoting patient engagement by integrating medical needs with Social Determinant of Health needs.

Responsibilities

• Meet or exceed daily outreach expectations towards phone calls, connections, and patient engagements
• Proactively outreach to both current and prospective patients via phone to educate them on care services and enroll them
• Use creative strategies and campaigns to empathetically engage patients in Strive’s care model
• Perform face-to-face patient outreach at homes or clinic visits
• Serve as a liaison between multiple service providers and assist with enrollment in services and community resources by delivering culturally competent care
• Use knowledge of local resources to manage Social Determinant of Health needs
• Administer health screening assessments (HRAs) to complete patient enrollment
• Review patient’s EMR to identify potential barriers to care and unmet SDoH needs
• Quickly build rapport with patients and external providers
• Identify situations calling for mandatory reporting and carry out mandatory reporting requirements by state requirements

Requirements

• 2+ years combined of related education, experience, or certification in the community health space
• Community Health Worker Certification
• 1+ years experience in enrolling patients or customers into a health/care program or promoting/selling services to end users
• Efficient and reliable transportation, including an active driver’s license
• Experience with phone outreach
• Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency

Qualifications

• 2+ years combined of related education, experience, or certification in the community health space
