About This Role

The Community Guide builds genuine, positive relationships with members in the community to improve their health and wellbeing, focusing on those with complex social needs and chronic health conditions. This role involves significant time in the field, utilizing an integrated technology platform for member interactions and documentation.

Responsibilities

  • Engage and work alongside clients from diverse ethnic backgrounds, focusing on those with complex social needs and chronic health conditions
  • Reach out to members to enroll them in Reema Health's care model
  • Assess member needs and support systems to create member-centered care plans
  • Establish trusting relationships, provide meaningful support, and follow-through to achieve wellbeing goals
  • Coordinate referrals and support appointment scheduling with various healthcare providers
  • Manage a caseload of 60-100 members through cold outreach and provide coverage for other Community Guides
  • Organize and prioritize workload to achieve monthly goals and outcomes
  • Actively listen, empathize, and maintain professional boundaries and confidentiality with members
  • Visit members in person in their homes, community, hospital, or other settings as appropriate
  • Utilize text, phone, email, and face-to-face interactions for communication based on member preference
  • Identify and connect members to community and health resources, including transporting members if needed
  • Document all work with members in the Reema App, including updating care plans
  • Comply with confidentiality of protected health information and HIPAA
  • Serve diverse members with dignity and respect, building relationships with community resources

Requirements

  • 3+ years of direct experience with target population/community
  • Knowledge of community resources
  • Experience with crisis planning, case management, boundaries, and personal safety in community work
  • Proficiency in critical thinking, time management, strategizing, and multitasking
  • Strong relationship-building skills
  • Ability to develop, adapt, and execute outreach and care plans
  • Proficiency with technology including Microsoft Word, Outlook, Excel, and Video Conferencing
  • Reliable access to transportation
  • Vehicle Insurance
  • Satisfactory driving record
  • Ability to pass a pre-employment drug screening and background check
  • Ability to lift and carry up to 25 pounds occasionally

Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences or other related fields, or equivalent related experience
  • Three or more years of direct experience with population or community to be served and knowledge of community
