This role involves scheduling patients for home health visits, ensuring consistent and continuous care, and optimizing clinician efficiency. The Clinical Assistant will manage workflows within the EMR system and ensure adequate staffing coverage for all schedules.
Responsibilities
- Schedule patients for the entirety of visit plans at the beginning of each episode, ensuring consistency and continuity of care.
- Ensure clinician efficiency by utilizing the productivity spreadsheet, updating it to identify opportunities and maintain appropriate staffing levels.
- Comply with Pending SOC standard operating procedures to process incoming referrals accurately and on time, striving for admission within 24 hours.
- Review and report daily to Care Coordinators and Clinical Managers all Pending SOCs that were scheduled but not admitted.
- Document updates on patient care and scheduling adjustments in communication notes.
- Schedule new hire clinicians in accordance with ramp-up expectations.
- Schedule resumptions of care following facility discharge and in alignment with Clinical Review Team notes.
- Work one major and one minor holiday per calendar year and provide availability for one weekend per month.
- Maintain patient and employee confidentiality in accordance with HIPAA regulations and agency standards.
- Schedule, reschedule, and manage patient visits across all disciplines in accordance with the plan of care, frequency orders, and episode timelines.
- Monitor underscheduled visits to prevent missed or delayed care.
- Ensure adequate staffing coverage for callouts, PTO, weekends, and holidays.
