Client Navigator

About This Role

The Client Navigator evaluates clients' behavioral, emotional, cognitive, spiritual, and social needs, facilitating individual and family sessions to develop and monitor treatment plans for those with substance use disorders, mental health concerns, and other behavioral health issues. This role ensures comprehensive clinical services and case management are provided in accordance with clinical policy and accreditation guidelines.

Responsibilities

• Evaluate behavioral, emotional, cognitive, spiritual, and social needs of clients and family members
• Consult with the treatment team regularly for the development of and progress towards treatment goals
• Facilitate individual sessions to develop treatment plans
• Facilitate all clinical services for clients including individual and family sessions and therapeutic groups
• Maintain clinical records for clients with appropriate documentation of clinical services and client program engagement
• Provide documentation and support for utilization review in a timely way
• Participate in aftercare plan development and outcome data process
• Provide case management services for clients and families through coordinating transportation needs and assisting in aftercare plan development
• Conduct Level of Care Assessments as required
• Complete all required trainings as designated by the company and accreditation/licensing entities

Requirements

• High school diploma or equivalent
• 1-2 years of experience in similar work (substance abuse and mental health field for degree holders)

Qualifications

• Preferred Bachelor’s Degree in human services or related field
• 1-2 years of experience in substance abuse and mental health field
