Provide compassionate personal care and support to hospice patients in their homes, working as part of an interdisciplinary team to ensure dignity, comfort, and peace for patients and families.
Responsibilities
- Provide personal care based on the individualized care plan (bathing, grooming, feeding, dressing, toileting, etc.)
- Maintain patient comfort and safety in their home environment
- Monitor and report changes in the patient's condition to the RN Case Manager
- Assist with transfers, mobility, and range-of-motion exercises
- Educate and support families on patient care techniques
- Document clinical notes accurately and on time
- Participate in team meetings and quality improvement initiatives
Requirements
- Active CNA certification in the state of employment
- Graduate of an approved Certified Nursing Assistant program
- Minimum 1 year of nursing assistant experience
- Current BLS/CPR certification
- Valid driver’s license
- Reliable transportation
- Car insurance
Qualifications
- Minimum 1 year of nursing assistant experience (hospice, home health, or community-based preferred)
