The Call Center RN will manage clinical coordination during on-call shifts, providing professional telephonic intervention to patients based on their Plan of Care. This role involves participating in the Interdisciplinary Team Process and offering leadership support to call center staff.
Responsibilities
- Manage clinical coordination during on-call shifts as initiated by algorithms and IDT members
- Provide quality professional telephonic intervention to patients based on the Plan of Care
- Participate and contribute to the Interdisciplinary Team Process
- Provide leadership support and guidance to call center staff for clinical and non-clinical issues
- Elevate complex issues to the Team Manager, AOC, or Hospice MD on call
Requirements
- Minimum of three years of hospice nursing experience
- New York State RN license
- NYS valid driver's license
Qualifications
- Minimum of three years of hospice nursing experience
Nice to Have
- Supervisory or leadership experience
- National Board Certification in Hospice & Palliative Care (CHPN)
Certifications
National Board Certification in Hospice & Palliative Care (CHPN) preferred or obtains within one year of hire
