About This Role

This LPN role involves providing quality care and supervision to residents in an Assisted Living and Memory Care community, focusing on a person-centered Social Model of Care. The LPN will coordinate with healthcare providers and ensure compliance with regulations.

Responsibilities

  • Assures the provision of quality services needed and/or desired by residents and in accordance with care plans and physician orders
  • Administers direct care as necessary
  • Provides day-to-day supervision of CRMA/PSS staff, providing coaching, mentoring, support and technical guidance
  • Communicates changes in resident health or behavior with Resident Services Manager, physicians, and families
  • Coordinates with physicians, pharmacies, and other healthcare providers
  • Maintains accurate and timely documentation in resident records
  • Ensures compliance with applicable regulations, policies and procedures; maintains survey readiness and proactively identifies areas for improvement

Requirements

  • Licensed LPN in the State of Maine
  • Current CPR certification or ability to obtain
  • Experience working with individuals living with dementia
  • Strong team approach and commitment to practicing positive communication
  • Demonstrated ability to train, mentor and coach others
  • Critical thinking and problem solving skills
  • PC knowledge and skills, including MS Word and Excel

Qualifications

  • 1-2 years of nursing experience preferred, with experience working with individuals living with dementia.

Nice to Have

  • 1-2 years of nursing experience (assisted living, long-term care, or geriatrics a plus)
