Provide outstanding person-centered intensive case management services to adults with severe mental health disorders and substance use treatment needs, assisting them in achieving stable mental health and community integration. This role is part of a multidisciplinary Assertive Community Treatment (ACT) team focused on reducing psychiatric hospitalizations.
Responsibilities
- Provide person-centered intensive case management services as part of the ACT Team
- Regularly meet with individuals in their homes, community, hospitals, crisis units, and clinics
- Deliver case management and rehabilitative skills training services
- Promote mental health stabilization and reduction of recidivism
- Increase individuals' awareness of and participation with community and natural supports
- Develop self-advocacy skills and assist with participation in independent behavioral health services
- Participate in frequent team meetings to share updates on recovery processes
- Work independently and on a team to prioritize needs during crisis situations
- Engage in regular meetings and training opportunities
Requirements
- Bachelor's degree in a Human Services related field (or 24 credits in Human Services)
- Education and experience in behavioral healthcare, intensive community treatment, and substance use treatment
- Valid Texas Driver's License and maintain approved driving status
Qualifications
- Bachelor's degree from an accredited college or university with a minimum number of hours equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, or early childhood education/intervention, or 24 college credits in a Human Services field if degree is not in Human Services
- Entry level with education and experience in behavioral healthcare, intensive community treatment, and substance use treatment. Preferred: previous experience with an Assertive Community Treatment program, behavioral health, and/or case management.
