The Annual Wellness Coordinator collaborates with Population Health, the Chief Medical Officer, and Vice President of Clinical Transformation to expand patient access to wellness services. This role focuses on providing tools for managing chronic conditions and enhancing the quality of life for current and prospective patients.
Responsibilities
- Ensure understanding of CMS Medicare Annual Wellness Visit requirements and workflows.
- Obtain baseline data on all patients, including medical histories, vital signs, health education, and lab tests during Annual Wellness Visits.
- Perform Telehealth visits with eligible patients.
- Collaborate with clinical staff to identify and resolve patient barriers to care in the community.
- Provide and facilitate competent healthcare of patients in the area of practice, adhering to standards of care and scope of practice.
- Lead individual and small group wellness education and counseling programs for eligible patients.
- Coordinate with clinical leadership to identify and develop initiatives for improving access to and quality of care or clinical outcomes.
- Participate in Community Outreach Programs and build relationships with other local healthcare organizations.
- Perform occasional telephone triage to assess patient problems, advise on treatment, and refer to other providers.
- Maintain up-to-date knowledge of Telehealth Care practices, clinical skills, standards of care, and CMS Medicare Requirements.
Requirements
- Current and Unrestricted LPN license in Ohio OR current and unrestricted Registered Dietitian in Ohio
- Current Basic Life Support (BLS) certification
- College Diploma or equivalent in Nursing or Dietetics
- Valid Driver’s License and Active Vehicle Insurance
- 2+ years as a Licensed Practical Nurse or Registered Dietitian
- Knowledge of EPIC or other EMR systems
- Working knowledge of local community
- Demonstrated ability to work with individuals on wellness education
- Strong written and oral communication skills
Qualifications
- College Diploma or equivalent in Nursing or Dietetics
- 2+ years as a Licensed Practical Nurse or Registered Dietitian
