Aftercare Coordinator

About This Role

The Aftercare Coordinator supports clients in their recovery journey by coordinating the next level of care and serving as a liaison with referral sources and families. This role ensures clients have clear, well-executed plans for continued recovery post-discharge.

Responsibilities

• Manage each client’s discharge plan, ensuring a seamless transition to the next level of care
• Collaborate with clinical staff and Banyan’s Outbound Team on client transitions
• Communicate with external agencies (VA, probation officers, attorneys, HR, EAPs) to ensure continuity of care
• Record all employment, legal, and medical-related interactions in the EMR (Kipu) for post-discharge planning
• Support AMA blocking efforts and help clients remain engaged in treatment
• Refer administratively discharged clients to alternative providers when necessary
• Keep families informed of discharge plans and provide consistent updates to referral sources
• Obtain consents, IDs, and financial documentation to support smooth transitions into care
• Collaborate with clinical and business development teams, participating in weekly case consultations
• Uphold confidentiality, regulatory standards, and a patient-first approach

Requirements

• High school diploma or GED
• Experience in behavioral health, customer service, or familiarity with 12-step programs
• Excellent written and verbal communication skills

Qualifications

• High school diploma or GED

Nice to Have

• Associate’s degree
• Familiarity with Salesforce and/or Kipu EMR
• Knowledge of local community resources
