Deliver comprehensive in-home medical care to patients, managing chronic conditions and developing individualized treatment plans. This role involves conducting assessments, exams, and medication reviews, ensuring coordinated care with other healthcare professionals.
Responsibilities
- Conduct in-home visits (minimum 25/week) to perform assessments, exams, and medication reviews
- Manage chronic conditions and develop individualized treatment plans
- Maintain accurate EMR documentation and comply with healthcare regulations
- Educate patients on disease management and healthy living
- Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients
Requirements
- Active, unrestricted licensure as a Provider in the state of Florida
- Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
- 1+ year of experience in primary care or a related field
- DEA license and BLS certification
- Strong clinical and diagnostic skills
- Proficiency in electronic medical record (EMR) systems
- Reliable transportation and clean driving record
- Ability to pass background check and 10-Panel Drug Screen
Qualifications
- 1+ year of experience in primary care or a related field such as family practice, general practice, or outpatient care.
