Advance Practice Provider

About This Role

Provide home-based primary care as an Advance Practice Nurse Practitioner or Physician Assistant, managing acute and chronic medical conditions for a panel of patients with physician oversight. Work within a multidisciplinary team to deliver comprehensive, personalized care to high-needs communities.

Responsibilities

• Perform comprehensive in-home assessments for identified patients to support chronic disease management
• Review current symptoms and identify diagnoses for care management and active medical treatment
• Complete physical examinations, medication reviews, vital checks, lab draws, wound care, and depression screenings
• Communicate assessment findings to PCPs to identify potential gaps in care
• Provide short-term and ongoing home-based care for patients with complex chronic medical conditions
• Coach team members, answer questions, address concerns, and oversee work quality
• Assist Medical Director/Clinical Leadership in reviewing APP charts for quality assurance and improvement
• Provide virtual and in-person consultation to team members during home visits
• Assist Management and Medical Directors with planning and execution of new team member training
• Train new team members on clinical processes and programs within individual's area of expertise
• Provide ongoing education and mentorship to team members

Requirements

• 3+ years of clinical experience as APRN/FNP/PA in primary care or home care setting
• Working knowledge of chronic care management and various medication options
• Ability to travel within the market to provide care to patients in their home
• Ability to evaluate and treat complex chronic conditions in the home setting

Qualifications

• Family Nurse Practitioner (FNP), Adult Nurse Practitioner with geriatric experience, Advance Practice Registered Nurse (APRN), or Physician Assistant (PA)
• 3+ years of clinical experience as an APRN / FNP / PA in the primary care or home care setting

Nice to Have

• Experience in Family Medicine, Internal Medicine, Geriatrics, Palliative Care, or home care
• Minimum of six months experience with Grace At Home
