Support individuals with mental illness or dual diagnoses in a residential setting to achieve greater independence, focusing on skill development and personal goal attainment within a clinically focused model.
Responsibilities
- Provide guidance and support to individuals in daily activities and living skills
- Implement procedures to ensure the health, safety, and well-being of individuals
- Maintain general cleanliness and safety of the physical site
- Advocate for individuals served and promote community integration
- Assist in developing individualized, person-centered treatment plans
- Play a lead role in the daily implementation of treatment plans
- Assist individuals to access and/or provide transportation
- Implement activities that create opportunities for the development of valued roles and personal relationships in the community
- Review and complete all required documentation
Requirements
- High School Diploma, GED or equivalent
- Valid Driver's License and acceptable driving record
- Reliability, willingness to learn, and open to new opportunities
